#1 29 June 1971 Soyuz 11
The only people who have ever died in space were on this mission, where the capsule lost all of its air during preparations for re-entry. The depressurisation caused all three men to die.
This was the first manned flight to arrive at the world's first space station. And sadly it was the last. A memorial was erected far east of Baikonur, in 2012 it was beyond repair, a restoration took place and the monument still stands.
#2 Columbia Shuttle Disaster
In 2003 the space shuttle Columbia disintegrated over texas killing all of the crew members on board. The disaster occurred because the left wing was hit by debris on launch. The bodies of the crew were later recovered in east texas. The debris was launched so far that people are still recovering it today. The area people have found the debris is 300 kilometres across lengthwise.
#3 January 28th 1986 Challenger Shuttle Disaster
Just as people were taking space flight for granted, the challenger shuttle exploded on re-entry. Killing everyone inside of it. And destroying the 2 Billion Dollar space ship. The families of the astronauts were paid 7.7million dollars in annuities. Space shuttle launches were halted for 2 years after this and space was once again considered a dangerous place.
#4 Soyuz 1
The first of the Russian 'Soyuz' crafts was never ready for launch. The ship was
plagued with technical issues. The soviet politicians thought it had to be launched.so they made sure it was. On the 23rd of April 1967, Vladimir Komarov stepped into the spacecraft that would be his tomb. This resulted in the ship's parachute not deploying slamming Vladimir into the ground at 150km/h, completely flattening the spacecraft.
#5 15 November 1967 X-15 failure
In 1967 Michael Adams died in a flight of the rocket plane x-15 on a flight to 81kilometres in altitude. The flight went well until the descent. The plane lost control when it was at a speed 5x that of the speed of sound. The craft broke up at 20km altitude. The pilot was awarded the honour of astronaut after death. As his flight went above 80.5 kilometres.
